Important Information
*Consultation visits include digital photos of before and after temporary makeup application, color and shape choices, medicial history form review, explanation of methods used and procedure application, healing instructions and any questions that need to be answered.
*All procedure fees include the consultation visit, 1st procedure and ONE touch up within 3 months of the first procedure.
*If makeup was done by another P.M.U practioner, pigments, needles, method & result information are requested at time of consultation. If any of the above is not available at this time you will be asked to sign a release form.
*Appointment confirmation or cancelations must be within 24 hours of scheduled date from client.
*Medical releases must be received the day of procedure. Any medical releases MUST be on the doctors prescription pad.

6 % New Jersey Sales Tax will be applied to each service

Color Booster fees are per feature and include touch up if needed
Color Corrections are $275 first visit / $100 ea. additional visit
Note: Color Corrections could take multiple visits
